Menstruation is a natural physiological process that occurs as part of the menstrual cycle in individuals with a uterus. At the center of this process lies the uterus—a hollow, muscular organ located in the pelvic cavity.

Each month, under the influence of ovarian hormones—primarily estrogen and progesterone—the inner lining of the uterus, known as the endometrium, thickens. This transformation prepares the uterine environment to potentially support a fertilized egg should conception occur. Estrogen drives initial proliferation of endometrial tissue, while progesterone stabilizes and further develops the lining during the luteal phase.

If fertilization does not take place, levels of estrogen and progesterone decline sharply. In response, the blood supply to the upper layers of the endometrium diminishes. Without hormonal support and adequate circulation, these nutrient-rich tissues begin to break down. The resulting shedding of the functional layer of the endometrium—accompanied by blood, mucus, and cellular debris—is expelled through the cervix and vagina. This outflow constitutes menstrual bleeding.

The myometrium—the smooth muscle layer surrounding the endometrium—also plays an active role. During menstruation, rhythmic contractions of the myometrium assist in expelling the shed tissue and controlling vascular flow. These contractions can sometimes cause cramping sensations commonly experienced before or during menses.

Thus, the uterus serves both structural and dynamic functions in menstruation: it hosts the cyclic preparation and rejection of endometrial tissue, responds sensitively to shifting hormone signals, and actively participates in the physical expulsion of menstrual fluid

After menstruation concludes, rising estrogen initiates renewal of the endometrium, beginning another cycle. Through this precise coordination of anatomy, physiology, and endocrinology, the uterus ensures reproductive readiness—and when pregnancy does not follow, enables regular, self-renewing cleansing via menstruation.
